The United Way of Central Indiana MEWA (the "Plan") operates as a Multiple Employer Welfare Arrangement (MEWA) registered with the Indiana Department of Insurance (IDOI). We are deeply committed to protecting the privacy and confidentiality of your Protected Health Information (PHI) under both the federal Health Insurance Portability and Accountability Act (HIPAA) and applicable Indiana state law (including Indiana Code § 16-39).

This Notice specifically applies to records maintained by the Plan, including health risk assessments, individual medical questionnaires, and benefits information collected from Indiana employers evaluating or applying for membership in this MEWA.
